Interface with the medical or surgical team in the hospital or office setting

Coordinate transition of care between healthcare facilities from healthcare facilities to home
Comprehensive medication review
End of life planning and support
Provide resources to prepare advanced healthcare directives (Healthcare Power of Attorney, Living Will, DNR, etc.)
Organize medical bills
Provide assistance with insurance and medical billing review, interpretation, and negotiation
Research skilled nursing and assisted living facilities

We do not tell you what to do or make decisions for you

Our Process

The first step is to have a preliminary phone consultation (free) where we will identify your needs and goals and let you know how we can help. The next step would be an initial consultation.

We start every relationship with an initial consultation, during which I will get to know you and your family members and learn what is important to YOU. We will discuss your medical situation, discuss your wants and needs, and answer any questions you may have. I will then work to create a personalized plan for you and identify any other resources you may need.

Professional fees for continuing services will be assessed on an hourly or package basis.

Services are not currently covered by insurance.
